I think your problem is cell F15. You have already filtered by Region. Change this to District = ${district}.
Another important point, but I don't think that this is the problem here. Under name, you should avoid capitals and spaces, the District and Regions contain capitals and spaces eg District Commune I. Change these to district_commune_i
Also in the relevant field you use eg ${niveau_enquete}='national' it is normally better to use selected(${niveau_enquete},"national")
