The error message appears when your choice names are not unique. There are more details in the docs here: https://xlsform.org/en/#choice-names
As described at the link, you can skip the warning by setting allow_choice_duplicates to yes on your settings worksheet tab. But WARNING, as the message says you may not be able to analyze your data properly if the duplication was not on purpose.
