Error message while uploading form on handphone: "unfortunately ODK collect has stopped"

Hello the ODK Community,

I have been working on a survey form since a few days and regularly checking it on my handphone as I made changes on the Excel form. However, since yesterday I am unable to upload the last version of the form on my hand phone (the form has been successfully uploaded on Ona before that). From internet research, I am guessing that this error appeared because there is a problem on my form but I really can't find the error even after having looked at the form many times.
I used the app log cat to help identify the issue (report attached).

Use ODK Validate to verify that the form is well-constructed.

Or use the XLSForm converter http://opendatakit.org/xiframe/ which runs the
form through Validate after it is generated.

Thanks for your advice, I did both things: install ODK validate and check my form with it and XLSForm Converter which identified some errors. I corrected everything and now both ODK validate and XLSForm do not detect errors, but still, I am unable to upload the form on my phone... or even see the webform from ODK's website (error message saying ODK collect has stopped...)

Could you please help with this?
I attach the updated version of my form.

I think there is some confusion...

ODK Aggregate installers are provided for you to set up your own website
for capturing and processing your own forms.

The https://opendatakit.appspot.com site is a demonstration site that we
provide for people exploring the capabilities of ODK Collect and the data
collection process. It is not a site on which you would place your own form.

This is different from FormHub, SurveyCTO and other groups that provide a
server where your data is captured and various levels of user and technical
support.

Using ODK Aggregate, it is easy to set up your own server on Google
AppEngine (and have them manage all the details). See

Once you have set up your own ODK Aggregate, you will be able to upload the
form definition to that server, and then be able to submit completed
surveys to that server. You should also consider restricting access to
your server via the Site Admin / Permissions tab (visible after you log
onto your server with the super-user username).

Dear Mitch,

Thanks for taking the time to answer this. I wasn't really confused about what is ODK Aggregate or FormHub, etc. I have conducted several surveys already using ODK and FormHub or Ona.
My problem is really that my survey form seems to have an error that I can't find because when I try to upload the form on my data collection device (smart phone), it says: error, ODK collect has stopped". And the same problem also happens when I try to view the form from One (Web view): there is an error that stops the process and I can't see the form. However, I uploaded it on my Ona account successfully...

I am sorry but I really don't understand the problem what is the issue with my form... I am able to upload it on Ona (my usual server) but then UNABLE to view it on my phone (using ODK collect). WHen I connect to the server and ask to get this form, it says: Unfortunately ODK collect has stopped. And an error happens also when I try to preview the form on Enketo or when I try to enter the web form: Error during transformation.

I really need to solve this issue.

This form works fine for me on ODK Collect 1.4.5 (1048).

Do you have the latest ODK Collect ?

If you are using the latest ODK Collect, please capture an error log
