Tried uploading this but this is the error i keep getting "The XLSForm could not be converted: The survey sheet is either empty or missing important column headers" can someone please have a look and tell me what is wrong?

Your survey tab needs to be survey (lowercase) not Survey.
You also need to delete a required column - it is duplicated.
Cell A4 needs to be date (lowercase) not Date.
Cells A7 and A11 needs to be integer (lowercase) not Integer.
You can't have relevance on line 7 that references the answer to a question on line 15 asked later in the survey. Also, line 7 references Additional_people but your question name is Additionalpeople.
