These features you can get for free through settings:
Logo: custom splash screen
Name: which name do you want to customise? You can create shortcut widgets to individual forms (which you can name freely) on the Android home screen.
Server: configure through QR code
For the rest, I suggest writing up which additional features you need in greater detail and post on the marketplace category.
