External itemsets

Collect Client 1.4 (1038)
Google App Instance
Handsets used:
Motorola Xoom running 4.1.2
Samsung Galaxy Tab2 7" running 4.0.3
Samsung Note 2 running 4.1.2

Some advice needed on an issue I've encountered using external itemsets.

I have an itemsets.csv containing multi-level address data, the structure
of which is Client, Area,Location,Premises. I've constructed two XML files
for two different survey's but using the same itemsets.csv for both. Both
definitions and their associate media (inc. itemsets.csv) uploaded to the
aggregate server with no problems.

I downloaded both survey types to a handset with no problems, a check of
the directory structure shows all files in the correct directories. I
selected one of the survey form types, selected/entered information both
from the itemset and direct, completed and saved. I then selected the
second survey type form, tried to load and the client stops. It doesn't
seem to matter which survey form is selected first, the second one always
stops the client. The first survey form using the external itemset can
still be run as normal, only selecting the second one closes the client.
All other survey forms not using an external itemset load and work as
expected.

The only way to reinstate correct operation of the second form is to delete
both forms from the handset and reload from the aggregate server.

The XML forms were constructed using the method indicated on
http://opendatakit.org/help/form-design/external-itemsets/

As both the survey forms run fine when downloaded individually I have
excluded form/data design issues so any thoughts as to what the problem
maybe would be welcome.

Sounds like a bug so I've filed it at

The best way for you to help the core team is to attach a set of forms
that show the crash and a crash log (see
https://code.google.com/p/opendatakit/wiki/CollectTroubleshooting for
instructions).
