Extracting media from zip folder

Dear Team
I am trying to show a image in note field type from zip folder attached in survey form. Is it possible if yes how plz guide me.

You need to reference the picture in the image field and not the note field in the survey sheet. You also need to include your pictures in the choices sheet by including a field called image. You can see some more information in the documentation.
