Failure to upload XLS Form

1. What is the issue? Please be detailed.
I am trying to upload my XLSForm but I keep on getting a message that ...The XLSForm could not be converted: There has been a problem trying to replace ${name} with the XPath to the survey element named 'name'. There is no survey element with this name.

2. What steps can we take to reproduce this issue?

There was a problem on the settings worksheet on instance_name

  2. What happens if you try and validate the form on https://getodk.org/xlsform/?
  3. What file size is your form?
  4. Are you able to share your XLSForm file here?

Where are you getting this error? Uploading a form to ODK Central or to the converter?
