I'm looking to build a form which it will show similar as my picture in the
attachment in ODK Collect. Would you mind to give some ideas or tools to do
that?

Thanks for your answer, so you mean that i need to write new widget for
this type of check box by myself, but i have a few question regarding to
build my own widget. Will JAVA Rosa include in this developing? Do i need
to understand Java ROSA deeply to build this widget or not? And can i do
some format, i mean by adding some colors or to text.?

I don't think you'll need to touch JavaROSA to get this sort of widget
in ODK Collect. Just need to add an appearance attribute. Follow the
example of the existing widgets.
Adding formatting to ODK Collect is not easy. ODK Survey has
formatting support already.
