Having issues uploading my excel survey sheet needed for trial

Hello,

my name is Michael, i need help. using this for this first time, uploading excel survey sheet (.xls) to the xlsform online. it says Error: The survey sheet is either empty or missing important column headers.

i know this has been asked before but i have followed all steps still not work.

Yes i am using a MAC.

Your sheet name should be survey (lowercase) not Survey.
