I do have questionnaires but I have encountered challenges in doing it.

The community can help best when you have specific questions. The ODK team, and volunteers, have put a lot of work into the documentation for the project:

If you don't know how to build forms so that you can convert what you have shared in those doc files into ODK forms please get familiar with https://xlsform.org/en/ and https://docs.getodk.org/form-question-types/ if you have any specific question please feel free to ask but I don't think anyone here will create the whole form for you.
