Hello All,
I want to conduct a survey for 100 health centres in 6 regions. Each health centre will fill out 4 seperate survey forms.
I want to create a summary Google sheet (in Google cloud) for each of the 6 regions–1 sheets summary per region. Each health centre will have 1 row that indicates whether a the each of the forms were submitted or not (1/0). At last, I would like to have a summary of the percentage of the forms submitted in each region.
I summary, I want to depict a dashboard that gives me this functionality. However, the summaries will be associated with the pre-selected health facility names.
