I have an ODK form, and when i uploaded it into Kobo toolbox, i challenged many problems. i used validator, but also was not enough helping. i need someone to fast check the file and help me to solve the problems.

I am using Kobo toolbox for humanitarian works.

i tried to check the validator. i did many check in the form itself. but really don't know where is the probelm

just checking the form and give me advices/help me to deploy the quesitonnaire.
