I am having two villages with same name in diffrent grampanchayat with diffrent unique code for every village but when its get uploaded in ODK form both the unique code are showing in the dropdown of unique code under one village name
Upload your form so we can see what the problem might be. My guess is that your choices sheet has rows you've hidden or rows farther down than you've scrolled.
Are you using 'village code' of selected village in choice filter, or are you using 'village name'?
If you have used 'village name' values in the name column of your choices list for villages and used that in filter column for your village-code list also; then this will happen.
