Lookup error from pre-loaded file

Dear All,

I have a problem about lookup with preloaded .csv file. Please see my
scenario below that data were I uploaded as pre-loaded .csv:

··· -------------------------------------------------------------------------------------------------------------------- REGION SR_Pcode TOWN TS_Pcode Bago (East) MMR007 Htantabin MMR007014 Bago (West) MMR008 Minhla MMR008009 Magway MMR009 Minhla MMR009013 Yangon MMR013 Htantabin MMR013006 -------------------------------------------------------------------------------------------------------------------- In my table, 2 pairs of town name are same spelling. But those are the *different location, different region, and different town_code* on the ground. Now, I really need to write syntax to autocomplete SR_Pcode in next hidden field (calculation row) when data collector select one of REGION, autocomplete TS_Pcode in next hidden field (calculation row) when data collector select one of TOWN.

I tried again and again about syntax in xlsform but I didn't get correct
result yet.
In ODK collect's first form, I chosen;
REGION: Bago (West)
TOWN: Minhla

In ODK collect's second form, I choose;
REGION: Magway
TOWN: Minhla

But I got wrong result after upload to server -- SR_Pcode are correct but
one of TS_Pcode is wrong . Please see below as result;

REGION SR_Pcode TOWN TS_Pcode
Bago (West) MMR008 Minhla
MMR008009
Magway
MMR009 Minhla MMR008009

WHY? Please help me with correct syntax to get correct result.
