You might want to check out episurveyor or frontlinesms for sending data via sms to your db.

Remember that if you have a big survey it is going to break the survey
into hundreds or thousands of SMS messages to send and then you need
to try to put them back together on the device. There is also no
guarantee of delivery from operators with sms so if even 1 of your
thousand does not arrive you do not have a complete survey.
This is quite apart from the cost issue involved with all this of course.Regards
