I would try removing the ODK application from the phone - then restart
phone or tablet....re-install the odk application and make all the
necessary settings then try getting blank forms and see if that work.

My next offer would be to backup your phone information and reset the phone
to factory settings - re-install odk and test it....that is however if you
really need it to work on that particular phone.

RE: phone -- did you mean the LG P500 Optimus One, running Android 2.x ?

To clarify:

ODK Collect stores forms and data on the SD Card.

If you connect your phone to your PC with a USB cable and "Turn on USB
Storage" on your phone.

The mounted drive that appears on your PC will be the SD Card.

ODK Collect stores all forms and data under the /odk directory on that
drive.

If you reformatted the SD Card, exchanged it with a different SD Card, or
deleted the /odk directory, then the forms you downloaded to ODK Collect
will be gone.

----------- If you try to run ODK Collect while you have USB Storage ON, it will crash or fail to start. You must "Turn off USB Storage" before you can start ODK Collect.

My form is run properly in other phone. I am working with Millennium
Promise in Millennium Villages. All our data collection in all sectors is
doing by phone.
We dont have problem with LG 615 or LG 720, only with LG P-500.

Just sharing...

I have used the ODK Collect on Bluestacks ( an android app player) that is
running on several of our laptops and desktops. I have not encountered any
issues so far. As long as the computer is able to install Bluestacks (
www.bluestacks.com). I am able to download forms from our server and send
the data to the server.

I would try removing the ODK application from the phone - then restart
phone or tablet....re-install the odk application and make all the
necessary settings then try getting blank forms and see if that work.
