I SHARED A FORM WITH GOOGLE DRIVE GOOGLE SHEET THE FORM WAS DOWNLOADED BUT THE PICTURE IN MEDIA DID NOT APEAR IN THE FORM

it's difficult to tell what was wrong not seeing that form. Could you share it with as as well?

Dont forget that the form results go into a spreadsheet, the image should be a link to an image in a folder in your google images. Look for the name of the .jpg file in the url link and search your google drive for that file perhaps?
