Where can i get the tutorial for designing a form?

Have you tried http://opendatakit.org/help/form-design/ ?

You have three options:

  1. use XLSForm - http://opendatakit.org/help/form-design/xlsform/
  2. use ODK Build - http://build.opendatakit.org
  3. or write the XML in long hand - see examples here:
    http://opendatakit.org/help/form-design/examples/

Best to to download sample forms here:
http://code.google.com/p/opendatakit/source/browse?repo=forms and study
them as well.
