I am currently working on an initiative where we are adopting ODK for coordinating our Post-Distribution Monitoring efforts.

ODK is a very adaptable and powerful tool and so could certainly be used for nutrition surveys. A good resource to review for designing such a survey might be the Guidelines for Conducting Nutrition Humanitarian Needs Assessment and Analysis.
