Please, is there a way to create a checking table (attached file: empty cells to check) into odk collect form?

Hi @Amal. This kind of table is only possible with selections. See the documentation. I realize it's not entirely clear how this relates to your example so you may like this example. I've filed an issue to improve the documentation at https://github.com/opendatakit/docs/issues/1195.

I would really like to see this expanded to text or number question types as described at Form design: grid of text or numeric input. If you have interest in that feature, please describe your use case in that thread.
