Labels disappearing when using media files

I think you'll need to either
[1] change label::English to just label
or
[2] change media::image to media::image::English

Notice that with your form as is, if you go to the option to change languages that there is both a "default" and "English" option. See the following post for some more details:

When testing this I had issues with media displaying correctly when there are language options in the survey if the media column(s) are not also localized. If you remove the ::English you should be fine. Or you can add a media column for each language. See below.
