Error: "Unknown question type 'select_one'."
please correct the error above from my xls form and explain to me how you did it i will appreciate if you reply ASAP. i have been trying all my effort and knowledge since 5 am yesterday to now.

you have problem in line number 5,7,8,9,28,29,31,34. correct it

The is an specific format that you should follow to use select_one questions, it is always select_one followed by a blank space and then the name of the correspondent list on the choices sheet.
In you example at cel A5 the name of the list is missing and the other select_one questions doesn't have a blank space between the select_one clause and the name of the list.
Also in your choices sheet the first column's title should be list_name and not list_names,
